COVID-19 VACCINE INFORMATION

First Choice Primary Care is a participating provider in COVID-19 vaccination efforts with the Georgia Department of Public Health. At this time, we are offering vaccinations to prioritized individuals according to the guidelines set forth by the GA Department of Public Health (below). If you are eligible to be vaccinated, please follow the steps below for registration information.

Due to high demand and constrained vaccine supply, COVID-19 vaccinations are by appointment only. If you are interested in being put on the waiting list please click the button below. We will only make appointments when the vaccine is available. Vaccination date, time, and location will be provided when your appointment is scheduled.
